Payment

Payment is due at the beginning of each session unless otherwise negotiated.  I accept all payment in the form of cash, check or credit.

Primary Insurance Plans Accepted

I am currently an in-network provider for the following insurances:

Premera - Regence - LifeWise

In-network billing requires that Premera, Regence, or Lifewise is your primary insurance plan.  If any of these are your secondary insurer, billing cannot be in-network.

Please check with your insurance company for your specific plan to determine if services are covered and what your deductible, co-pay, or expenses might be.

I accept and courtesy bill all major insurance companies as an Out-Of-Network provider.  Please check with your insurance company to determine your out-of-network benefits.  It is likely your insurance will cover a portion of the cost.

Cancellations

Please provide at least 24 hours notice if you need to cancel or reschedule an appointment to avoid being charged for the session.  Late cancellations (0-23 hours of scheduled session) will be charged half the session fee.

No call/No show

Full session fee is charged for missed sessions without cancellation
